Nurse Information

Head Lice

When school starts, and after Christmas break each year, we will be checking for HEAD LICE! Please check your children prior to school starting and treat them if necessary. We will be checking as many heads as possible on the first day. With a great attempt we can control the amount of Head lice to below the amount we have had in past years. Our NO NIT POLICY on head lice is in effect, children with nits (eggs of the louse) will be sent home. They do not ride the school bus home. If you treat your child a day or two before school starts send a note to tell me so that we do not call you. All nits must be removed prior to the child's return to school.

Physical exam and immunizations

New Entering Students, and Current Students
The State of Wisconsin requires that all students have the following immunizations prior to school entrance in order to be compliant with its immunization law:

(1) DTP/DTaP/DT vaccine for children entering Kindergarten: Your child must have received one dose after the 4th birthday (either the 3rd, 4th, or 5th) to be compliant. (Note: a dose 4 days or less before the 4th birthday is also acceptable).
(2) DTP/DTaP/DT/Td vaccine for students entering grades 1 through 12: Four doses are required. However, if your child received the 3rd dose after the 4th birthday, further doses are not required. (Note: a dose 4 days or less before the 4th birthday is also acceptable).
(3) Tdap means tetanus, diphtheria and acellular pertussis vaccine, recommended for adolescents. If your child received a dose of tetanus or diphtheria containing vaccine such as Td within the past 5 years, Tdap is not required.
(4) Polio vaccine for students entering grades Kindergarten through 12: Four doses are required. However, if your child received the 3rd dose after the 4th birthday, further doses are not required. (Note: a dose 4 days or less 1 before the 4th birthday is also acceptable).
(5) The first dose of MMR vaccine must have been received on or after the first birthday (Note: a dose 4 days or less before the 1st birthday is also acceptable).
(6) Var means Varicella (chickenpox) vaccine. A history of chickenpox disease is also acceptable.
(7) Students 13 years of age or older without a prior history of chickenpox disease, or who received their first dose of varicella vaccine at 13 years of age or older, are required to receive 2 doses of varicella vaccine.

If you choose not to have your child immunized for health, religious, or personal reason, there is a form called the Student Immunization Record Sheet to sign. Please call the nurse for this form.

NOTE:

  1. MMR. The first dose of the MMR must have been received on or after the first birthday
  2. *DPT. For entering K. the 3rd, 4th, or 5th dose needs to be after child's 4th birthday.
  3. DPT and Polio For grade 1-12. Four shots are required. However if child's received the 3rd dose after the 4th birthday, further doses are not required.

Kindergarten
Parents of children entering 4 or 5 year old Kindergarten please be reminded that your child needs to have a physical exam.
Immunizations must be brought up to date also. State laws mandate this so please try to have this taken care of before school starts. You may mail this information to school addressed to the nurse before September 2008.
Parents of children in Grade 6, verification of having Chicken Pox disease or having the Chicken Pox shot will be needed in this grade.

Milk or Dairy Product Allergies

If your child is lactose intolerant or allergic to dairy products:
Please send a note from your child's physician stating this fact.
DPI requires that we have this note on file for our lunch meal program.

Medication

Should your child need to be on medication while in school and you would like me/or staff to administer, there is a policy to follow:

  1. Medicine sent to school must be in the original medicine bottle and labeled with the child's name, the drug, its dosage, the time to give, and the physician.
  2. A parent/guardian signature is needed on the school Medication/Procedure form. Please obtain this form from the school, your family doctor or download and print the .pdf version of the form.
  3. If the medicine is prescribed by a physician then I must have the physician signature on the order for me to give medication. Please obtain this form from the school, your family doctor or download and print the .pdf version of the form.

Medication Administration Procedures.

Written Instructions and consent. School personnel will administer no prescription medications unless and until the folowing written statements are provided the principal:

  1. Parental Medication Consent - authorizing school personnel to give the medication in the prescribed dosage and authorizing school personnel to contact the physician directly.
  2. Physician Medication Administration instructions - providing guidelines to school personnel regarding the administration of the prescribed medication and identifying specific conditions and circumstances which contact should be made with the physician concerning the condition or reactions of the student to the prescribed medication.

The length of time for which medication is to be administered shall be specified in the written instructions from the prescribing physician. Any change in dosage, time to be administered or discontinuance of administration must be in writing at the request of the physician

Only limited quantities of any medication shall be kept at school. Said medications shall be kept in a safe place not accessible to students and checked out only by school personnel authorized to administer medications. If you have any question please call school nurse.
